How to Apply for Services

Getting help is straightforward. Follow these steps to access emergency shelter and housing assistance through FIELDS of Green County:

Call the Housing Intake Line
Contact us at (608) 328-9640 to speak with a trained intake specialist. Lines are open Monday through Friday, 8:00 AM - 4:30 PM. Leave a message after hours and we'll return your call.

Complete Initial Screening
Our staff will ask questions about your current housing situation, needs, and circumstances. This helps us determine the best services and resources for you.

Coordinated Entry Assessment
You'll complete a standardized assessment that helps prioritize services based on your vulnerability and needs. This ensures those in greatest need receive assistance first.

Receive Shelter Placement or Referral
Based on your assessment, you'll be connected to emergency shelter at FIELDS or referred to other appropriate housing resources and support services in the community.

Work with Case Management
Once placed, you'll work with case managers to develop a housing plan, access supportive services, and work toward stable, permanent housing.
