What is Coordinated Entry?

Coordinated Entry (CE) is a comprehensive system designed to create a more equitable and streamlined process for connecting and allocating our community's limited homeless-related resources. Through Coordinated Entry, households in need of assistance are identified, assessed, referred, and connected to housing and assistance based on their strengths and needs.

No matter where you seek help in Green County, you'll be connected to the right resources. The Coordinated Entry system ensures that anyone experiencing homelessness or at risk of homelessness receives a coordinated, person-centered response.

How Coordinated Entry Works

1. Access & Identification
Contact any participating agency or call our Housing Intake Line. You'll be connected to a trained professional who can assess your situation and needs.
2. Assessment
Complete a standardized assessment that evaluates your housing situation, barriers, and needs. This helps prioritize assistance based on vulnerability and need.
3. Prioritization & Matching
Based on your assessment, you'll be prioritized for available resources and matched with the most appropriate housing intervention and support services.
4. Referral & Connection
Receive referrals to housing programs, emergency shelter, and support services that best meet your individual needs and circumstances.
5. Housing Placement
Work with case managers to secure housing and access supportive services that promote long-term stability and self-sufficiency.
6. Follow-Up
Receive ongoing support and follow-up services to ensure housing stability and connection to community resources.
